Predictors of contrast-induced nephropathy in ST-elevation myocardial infarction patients undergoing primary percutaneous coronary intervention

چکیده
Abstract Background Contrast-induced nephropathy (CIN) is a serious complication with primary percutaneous coronary intervention (PPCI). We aimed to study the different predictors of CIN and determine cutoff point contrast volume (CV)/creatinine clearance (CrCl) applicability CHA2DS2-VASC score in prediction after PPCI ST-elevation myocardial infarction (STEMI) patients. Four hundred patients presented STEMI eligible for PCI were included study. Patients GFR < 30 ml/min excluded from Results Fifty-four (13.5%) who developed older (64.20 ± 13.16 vs. 55.80 10.58) had higher prevalence diabetes mellitus (DM), hypertension (HTN), female gender than those without CIN. They also Killip class lower hemoglobin (HB) level ( P 0.05) compared no The incidence was (85.8%) low-risk Mehran group 14.2% moderate-risk group, all high very <0.001). Multiple logistic regression showed that old age (OR= 1.06, 95% CI= 1.02–1.11, 0.001), sex 3.1, 2.65–6.99, = 0.02), (OR=2.48, 1.98–6.24, 0.01), CV/CrCl > 2.8 (odds ratio=1.45, 1.22–2.01, 0.03), 2 ratio=1.90, 1.76–2.11, P= 0.04) Conclusions Old age, sex, score, CHADS2-VASC 2, underwent PPCI.

Smoker's Paradox in Patients With ST-Segment Elevation Myocardial Infarction Undergoing Primary Percutaneous Coronary Intervention.
BACKGROUND Prior studies have found that smokers undergoing thrombolytic therapy for ST-segment elevation myocardial infarction have lower in-hospital mortality than nonsmokers, a phenomenon called the "smoker's paradox." Evidence, however, has been conflicting regarding whether this paradoxical association persists in the era of primary percutaneous coronary intervention. METHODS AND RESULTS...
متن کاملComparison of the Success Rate of Treatment with Primary Percutaneous Coronary Intervention PCI versus Thrombolytic Treatment in Patients with ST-Elevation Myocardial Infarction in Local Hospitals in Iran
Background and Objective: Acute myocardial infarction (MI) is caused due to coronary artery occlusion and divided into two forms of ST-elevation (STEMI) and non-ST-elevation (NSTEMI) myocardial infarction. This study aimed to determine the success rate of treatment with primary PCI (percutaneous coronary intervention) versus thrombolysis in the establishment of perfusion and to evaluate the sho...
متن کاملAssociation of Syntax Score II with Contrast-induced Nephropathy and Hemodialysis Requirement in Patients with ST Segment Elevation Myocardial Infarction Undergoing Primary Percutaneous Coronary Intervention
BACKGROUND AND OBJECTIVES Contrast-induced nephropathy (CIN) is a common complication of primary percutaneous coronary intervention (pPCI) and is associated with high mortality and morbidity and long hospital stay in patients with ST elevation myocardial infarction (STEMI). The Syntax Score (SS) has previously been studied in STEMI patients, and it was associated with increased CIN development ...
متن کاملContrast-Induced Nephropathy in Patients Undergoing Percutaneous Coronary Intervention
BACKGROUND Contrast media are being widely applied for both diagnostic and therapeutic purposes. This has resulted in increasing incidence of contrast-induced nephropathy (CIN). METHODS We aimed to investigate the value of baseline serum IL-6 concentrations in predicting CIN before the rise of serum creatinine (SCr) in patients undergoing percutaneous coronary intervention. Seventy four Cauca...
متن کاملEvaluation of ejection fraction in patients with acute myocardial infarction undergoing percutaneous coronary intervention
Introduction: Percutaneous coronary intervention (PCI) is a reperfusion strategy to increase life expectancy and ejection fraction (EF) in ST-segment elevation myocardial infarction (STEMI) patients. We investigated the effect of location, severity, type of lesion, number and type of vessel involved and time of angioplasty on the increase in EF in STEMI patients undergoing primary PCI (P-PCI) a...
